Purchase a Wood Fishing Rod and at least five Worms (ten is recommended). Crucially, you *don't* equip the bait; simply keep the Worms in your inventory. Each successful catch (or even junk!) consumes one Worm. A crucial detail: having at least five Worms seems essential for catching anything. Consider placing them in your hotbar for easy monitoring.
How to catch fish in Rune Slayer

Watch the bobber; when it ripples (once or twice), click M1 again to reel in your catch. Success isn't guaranteed; you might catch nothing, or even junk, but Simon counts it all towards your quest.
